The Community Health Representative (CHR) works to improve the overall health of our Nation by ensuring members’ health needs are addressed. They coordinate and deliver community-based health promotion and prevention programs to achieve health through wellness.
Duties and Responsibilities
- Plans, coordinates and implements all the health promotion activities for the community, including, but not limited to: the baby welcoming, the health fair, men’s and women’s wellness events, diabetes awareness, healthy eating, food security and physical activity as part of wellness.
- Coordinates and delivers a wide range of community-driven and culturally relevant health promotion and chronic disease prevention activities with the Health Care Nurse.
- Encourages and supports community initiatives and activities to facilitate greater community education, responsibility, and involvement in health promotion.
- Plans, promotes, implements and evaluates community health education programs, including sexual health education, chronic disease, children’s oral health initiative (COHI), optometrists’ visits and other proactive health education programs.
- Keeps informed and up to date on health services and benefits provided by Saulteau First Nations, FNHA, Health Canada, and Treaty 8.
- Provides members with information on various health services and providers.
- Supports and advocates on behalf of members with external agencies such as local health care providers.
- Engages individual and community input in determining health care needs and relays this information to the Operation Manager for setting health care priorities and program delivery.
- Encourages and supports community initiatives that facilitate greater community responsibility and involvement in health care.
- Assists individuals and families to recognize their own health needs and encourages the taking of appropriate action to meet these needs.
- Manages and monitors budgets and expenditures relating to various CHR events and programs.
Other duties and responsibilities
- Oversees the annual planting, gardening, and maintenance of the community garden.
- Participates in various committees within the Nation.
- Cross trains in health center reception and provides coverage.
Qualifications and Skills
- Grade 12 or equivalent with a health education background relevant to the position.
- Completion of the Aboriginal Health Community Administration Program certificate is considered an asset.
- Possesses cultural awareness and sensitivity.
- Ability to build trust, serve as a positive role model and maintain confidentiality.
- Demonstrate a healthy lifestyle and be a positive role model for the community.
- Experience with Microsoft Word and Outlook.
- Ability to write or complete required reports.
- Good verbal and written communication skills.
- Valid driver’s license, and use of a reliable vehicle for work-related responsibility.
- Ability to pass a Vulnerable Sector Police Information Check.
- COVID -19 Vaccine is a requirement.
- Work is performed primarily in the office with hours of work set as 8:00 a.m. to 4:30 p.m. Monday through Thursdays.
- Travel may be required within SFN community and surrounding community
